Massachusetts Driver’s License or Massachusetts State ID, U.S. Passport, or Military ID with proof of
Massachusetts residency. If you are a veteran, please be sure to upload proof of service such as a Veteran ID
or DD-214 for the discount to be applied to your invoice. If you have a hardship, please upload proof of
financial hardship for the discount to be applied. Your invoice is available for payment online in your patient
portal. You have the ability to process the payment yourself at any time from within the portal, or our staff
will complete the payment on your behalf the day of your appointment. If you are denied for any reason, we
will process your refund right after your visit.
Step 4: It’s time for your telemedicine visit! You will receive a reminder email and text prior to your visit. We
are doing visits using telemedicine, which requires a computer, tablet, or smartphone. You can easily
download an app to connect with the practitioner, or if you are having trouble connecting, they will reach out
by telephone. If you need any assistance in connecting to your visit online or would prefer a telephone call,
please call us at 617-420-7890. Once connected, the practitioner will review your medical history with you
and discuss how to integrate or supplement cannabis into your wellness routine.
Step 5: Once your visit with the practitioner is complete, they will enter your new certification into the
Cannabis Control Commission Portal, valid for a full year. We will send you a thank you email with
instructions on how to proceed. If you are a new or expired patient, this email will include a temporary ID,
ready to use immediately at dispensaries.
Whether you are a new or renewing patient, please make sure to complete the annual state registration
process to receive your hard copy Medical Marijuana Patient Card. If you need any assistance at all, please

Step 6 (Optional): You have the option of designating a personal caregiver to assist you with your medical use
of marijuana. After your registration has been approved by the MA Cannabis Control Commission, you may
name up to 5 personal caregivers.
